I don’t have proof of eligibility
If you do not possess any of the eligibility proof listed, please follow the steps below:
- Have Section E completed by an authorized health or social services professional.
- Check the box “I do not have proof of eligibility” in the online form.
- Complete Section D of the online form.
- Upload Section E in the required field of your online application.
- If you encounter difficulties uploading, you may also print and fill out Section D, and send both Sections D and E to us by email or by mail.
Important: Sections D and E of the form can replace the proof of eligibility if you do not have one.
