Provide proof of eligibility
To complete your application for the Companion leisure card (CAL), you must provide one proof of eligibility from the list of accepted documents below.
If you are applying for a child aged 5 to 18 years old:
- Canada Child Benefit Notice including the Disabled Child Benefit – PEH (Canada Revenue Agency).
Example - Notice of the supplement for handicapped children included in the family allowance (Retraite Québec).

Other Example - The adapted transport card issued by a representative organization for adapted transport which applies the Eligibility Policy of the Quebec Ministry of Transport.

If you are applying for an adult:
- Notice of assessment for severe and prolonged impairment in mental or physical functions (Revenu Québec, refundable credit).
Example - Disabled Person Tax Credit (Canada Revenue Agency).

Example - The Quebec accompanying card for intercity bus service (Bus Carriers Federation).

Government documents provided as proof of eligibility must contain:
- The name of the applicant;
- The date of the document;
- The sentence clearly indicating access to the program.
