Submit a New Application
To obtain your Companion leisure Card (CAL), you must submit a complete application that includes:
- The CAL application form.
- One proof of eligibility from the accepted list OR Sections D and E of the CAL form.
- A recent photo of your face that meets the specified criteria.
How to Submit Your Complete Application
